This site has been established to assist healthcare professionals in optimising anticoagulation for the prevention of stroke in Atrial Fibrillation (AF) through both improving anticoagulation services and providing assistance and visual aids to help during patient consultations.

The patient consultation section can be used during a consultation with a patient to help you establish and explain;

  • a patients' individual stroke risk (using CHA2DS2-VASc)
  • how anticoagulation can reduce this risk
  • a patients' bleeding risk with and without anticoagulation (using HAS-BLED)
  • how these risks compare to other everyday risks

At the end of the consultation, you will be able to print and provide the patient with a useful risk sheet (based on their scores) to help them consider or understand the decision to initiate anticoagulation therapy. In this section, you will also have access to a toolkit which includes key factors which may influence the decision to initiate anticoagulants and frequently asked questions regarding the oral anticoagulants available.

The Quality Improvement in AF section will provide you with information, tools and templates aimed at assisting you and your team in assessing and improving your anticoagulation service through a quality improvement process.
